We’re looking for examples of teams, services or projects that are working to support the aims and principles of our Integrated Care Strategy.
Every month, we’ll do a ‘shout-out’ to a team and share this on our social media channels and in our newsletters.
You can nominate your own team or project or send in a nomination on behalf of someone else. Please make sure you have their permission first.
