Care in my area
Place is our way of describing joint working within each of our four local authority boundaries of Nottingham and Nottinghamshire.
By working together, we know that we can drastically improve population health. We do this by combining the services of GPs, social care, mental health, hospital, community and voluntary organisations into a more joined up system, focusing on a person’s individual needs and providing them with the support to manage their own care.
Place is where shared decisions are made about how the local health and care resources can be used to improve the health of the population and deliver more joined up care.
