City clinical and professional model design update
Key partners involved in the clinical development of Nottingham’s Integrated Neighbourhood Teams (INTs) came together in November to shape best practice and map out the patient journey ahead of the programme’s launch across the two pathfinder neighbourhoods in Bestwood & Sherwood and Nottingham City East.
Chaired by Dr Andy Foster, the session brought together clinicians, operational leads, primary care representatives, CityCare colleagues, mental health services, community and voluntary sector partners, and health and care professionals who are central to rolling out INTs in the pathfinder areas. The group explored how the emerging clinical pathway can best support people to receive joined‑up, proactive and personalised care.
Discussion focused on several core elements of the model, including:
• consent and information‑sharing
• clarity of roles and responsibilities
• a consistently patient‑centred approach
• communication and accessibility
• data collection and process design
Ciara Stuart, Assistant Director at Nottingham Place Based Partnership, described the session as hugely positive, highlighting the shared commitment to delivering high‑quality, patient‑focused INTs.
The group also discussed the importance of a single, portable care record that follows a person across all providers, ensuring seamless care and reducing duplication. Ensuring individuals have meaningful input into shaping their own care was a key theme, as was the vital role of VCSE organisations in supporting neighbourhood‑level delivery.
The draft INT clinical pathway for the two pilot neighbourhoods will now be refined ahead of a further review by key stakeholders on 20th of January.
