About Primary Care Networks (PCNs)
Primary Care Networks (PCNs) will bring together a range of local organisations and groups, including community services, social care and the voluntary sector to offer coordinated health and social care to their local populations.
A key focus of the PCNs will also be around helping residents to look after their own health. By intervening at an earlier stage and equipping them with the skills to manage their health, there will be less likelihood of them needing treatment.
The South Nottinghamshire PBP is made up of three localities. Within these localities, there are one or more Primary Care Networks, which, in some cases, are further broken down into administrative ‘neighbourhoods’.
The South Nottinghamshire ICP Board is part of the Nottingham and Nottinghamshire Integrated Care System (ICS).
