Our ambition is for people living in Nottingham and Nottinghamshire to live longer, happier, healthier and more independent lives. Our Clinical and Community Services Strategy sets out how we will provide care in the future to achieve this. It aims to develop a model of care that is delivered by the whole health and care system as a whole, being more proactive, focusing on prevention and early intervention and providing services closer to people’s homes.

The Clinical and Community Services Strategy model is about the whole system working together to support people throughout their lives, helping them to live healthy lives and supporting them when they become ill.
The strategy is the start of a long-term process of transformation and is driving a programme of service reviews across the health and care system. These reviews are bringing together doctors, nurses and other health professionals to work side-by-side with patients to look at how the way we deliver care can be improved.
We have already completed 5 in-depth service reviews. These have looked at how to transform care for stroke; respiratory conditions; frailty; maternity and neonatal services and children and young people.
