Transformation programme
Organisations from across the Integrated Care System (ICS) have committed to a major programme to transform the way we deliver health and care to people in Nottingham and Nottinghamshire.
The transformation programme aims to improve health and care services for local people and make them more sustainable for the future. We want to arrange services in a more joined up and efficient way that works better for people who use them and is also less expensive to run.
The areas covered by the transformation programme are:
- ICS strategic workforce plan and delivery
- Frailty agenda
- Urgent and emergency care transformation
- Digital transformation
- Community transformation
- End of life / hospice care
- Best value services
- Provider Collaborative including elective pathway redesign
Process and timescales
Each workstream has a Senior Responsible Officer (SRO) who will lead the transformation and help bring together partners from across the ICS.

How to get involved
All staff working in our health and care system will have a part to play in the transformation work. Some people will be involved at an earlier planning stage, helping to shape the way services are delivered. As the transformation work progresses, other people may need to work differently, or in a more joined up way, to help deliver change.
Here are some ways you can support:
- We want the right people from across the health and care system to be involved in the transformation work. If you think you can support with the any of the workstreams, please email nnicb-nn.systempmo@nhs.net
- Clinical Design Authority: The Clinical Design Authority is recruiting clinicians, non-clinicians, healthcare professionals, IT professionals, and other stakeholders to support with system transformation work. People can sign up as bank staff and will be paid for the support they provide. This covers people working in Adult Social Care and Public Health, as well as NHS staff. - If you work in primary or secondary care, there are already ways you can support the transformation work by ensuring you use the Value Based Commissioning policy when commissioning services. This will ensure that we are commissioning services in the most equitable way for all our citizens and achieving the best value for money.
