Launching Two New Integrated Neighbourhood Team Pathfinders in Nottingham City

Neighbourhood-based care in Nottingham City has taken an important step forward with the launch of two Integrated Neighbourhood Team (INT) pathfinders in Bestwood & Sherwood and Nottingham City East. These new teams bring together health and care professionals to deliver more coordinated, proactive support for local residents.

The INT model unites a wide range of roles — including GPs, Care Coordinators, Social Prescribing Link Workers, Community Geriatricians, Community Nursing teams, Community Matrons, Mental Health Practitioners, Social Workers and Nottinghamshire Hospice — to work as one team around shared patient needs. This approach aims to identify people who would benefit from more joined‑up, personalised care, particularly those with complex or deteriorating health. The teams initially focus will be working with people over 55 years old who are living with frailty to reduce with the most complex needs, who place the greatest demand on healthcare services and would benefit most from an integrated neighbourhood team approach.

First Multi‑disciplinary Team Meeting

On 16 December, colleagues from across both neighbourhoods came together for the first multi‑disciplinary team meeting. The session tested the emerging clinical model, walked through the patient journey, and reviewed the first group of patients identified for proactive support. Staff welcomed the opportunity to collaborate more closely.

Next Steps

Over the coming weeks, the pathfinders will continue refining processes, strengthening communication across organisations, embedding proactive case reviews, and gathering feedback to shape the wider rollout of INTs across Nottingham City. The teams have already begun supporting their first cohort of patients, ensuring care is coordinated, personalised, and focused on what matters most to each individual.

These early developments lay strong foundations for more integrated neighbourhood‑based care, with the commitment shown on 16 December highlighting the shared ambition to make this model a success across the city.
